State profile · Indiana
Every figure here carries the Indiana document behind it.
The short version: a Indiana single-trip permit starts at $20 before bands, overweight and the transaction charge; an escort is first required at over 12'4" wide; and the load becomes a superload at a gross weight the state does not publish. Everything below carries the Indiana document it came from, that document's own revision date, and the date we read it.
The jurisdictions this one borders, each chip carrying the fact that decides whether that crossing is the expensive one — so the row is itself a small comparison. A multi-state lane is the actual job, and from here it is one click per state rather than a return to an alphabetical index.
Anything over one of these needs a permit. Every value links to the document that sets it.

| Single axle | 20,000 lbrev. · read IC 9-20-4: "an axle weight in excess of twenty thousand (20,000) pounds". A separate limit of 800 lb per inch of tire width measured between the rim flanges also applies and is not modelled, because a quote does not collect tyre width. |
| Tandem axle | 34,000 lbrev. · read IC 9-20-4: "(A) Thirty-four thousand (34,000) pounds total weight. (B) Twenty thousand (20,000) pounds on an individual axle in a tandem group." |
Not one number per dimension — the whole staircase. Each row is a condition the state actually publishes, what it requires when that condition holds, and the clause it comes from. Where a rule requires escorts and the source states no number, the row says that in the source's own words rather than reporting no requirement.

| width over 17' | 1 police escort in front1 police escort behindEscorts required; the source states no number — at least 2. Published as: “At least front and rear police escorts are required; required slowdowns may necessitate more at the ISP’s discretion.”Cannot be reduced to a number: Indiana requires "at least front and rear police escorts" over 17 ft wide, and "required slowdowns may necessitate more at the ISP's discretion". The rates ARE published and are citable: the Indiana State Police superload escort agreement effective 1 March 2019 sets a sworn trooper at $43.00 per hour and a motor carrier inspector at $34.00 per hour, each escort must include at least one sworn trooper, and hours are computed round trip from the officers' homes, with vehicle mileage at Indiana's approved state rate of $0.64 per mile from 7 August 2026. The hours, the officer count and the officers' home locations are all set by ISP on the day, so no police-escort amount is included in the permit total. The 2019 agreement is more than three years old and remains the document ISP links. |

| gross weight from 200,000 lb (exclusive) to 250,000 lb (exclusive) and not (Has the issued permit confirmed that the route uses only interstates and requires no slowdowns?) | Escorts required; the source states no number — at least 1. Published as: “An ISP escort is required unless the route is interstate-only, has no required slowdowns, and the vehicle is under 250,000 lb GVW.”Cannot be reduced to a number: Indiana Form M-204S requires an ISP escort over 200,000 lb unless all three stated exception conditions are satisfied. Weight alone resolves the under-250,000-lb condition, but the approved route and required slowdowns are permit facts. Until those facts are confirmed, police remains unresolved and no officer count or cost is invented. |

Published as: “The M-204S exception applies only when the vehicle weighs less than 250,000 lb GVW.”Cannot be reduced to a number: At 250,000 lb or more, the published exception cannot be satisfied because it expressly requires the vehicle to weigh less than 250,000 lb. ISP must assign the officer count, hours, vehicles and travel; no police amount is invented. |

| Single-trip base | $20rev. · read The July 2026 DOR fee sheet prints "$20.00 + $0.35 per mile for vehicles up to 108,000 lbs." and "$20.00: up to 95' in length, 13'6" in height, 12'4" wide" — the same $20 reached from the overweight and the oversize side. |

| within 12 ft 4 in wide, 13 ft 6 in high and 95 ft long — no increment above the $20 base fee | $0rev. · read 105 IAC 10-1.5-3(b)(1): "Twenty dollars ($20) for a vehicle with dimensions not more than: (A) one hundred forty-eight (148) inches wide; (B) one hundred sixty-two (162) inches high; or (C) ninety-five (95) feet long." That $20 IS the base fee held in `permitBaseFeeUsd`, so this band adds nothing — the two together are the published $20 total. |
| over 12 ft 4 in up to 16 ft wide — $10 above the base fee | $10rev. · read 105 IAC 10-1.5-3(b)(2): "Thirty dollars ($30) for a vehicle with dimensions that exceed: (A) one hundred forty-eight (148) inches wide ..." — $20 base plus this $10 increment. Bounded above by the superload dimensions, where the $40 band begins. |
| over 13 ft 6 in up to 15 ft high, within 12 ft 4 in wide — $10 above the base fee | $10rev. · read The same $30 band reached on height alone — "(B) one hundred sixty-two (162) inches high". Bounded at the $20 width ceiling so it cannot also match a load already covered by the width row above; two matching rows with different bounds would be read by the resolver as two sources disagreeing, which they are not. |
| over 95 ft up to 110 ft long, within 12 ft 4 in wide and 13 ft 6 in high — $10 above the base fee | $10rev. · read The same $30 band reached on length alone — "(C) ninety-five (95) feet long". The July 2026 DOR fee sheet prints the same window as "between 96' and 110' in length". |
| over 16 ft wide — superload; $20 above the base fee | $20rev. · read 105 IAC 10-1.5-3(b)(3): "Forty dollars ($40) for an oversize/overweight superload", and the rule's superload definition is "more than fifteen (15) feet high; more than sixteen (16) feet wide; or more than one hundred ten (110) feet long" — $20 base plus this $20 increment. A load in this band is a superload on the rule's own reading and the engine will refuse to price it; the row exists so the schedule is complete. The DOR documents describe this $40 row incoherently — the July 2026 fee sheet prints "$40.00: up to 95' in length, 16' wide, 15' tall and 80,000 lbs." while the undated DOR fees webpage prints "over 110' length, 16' wide, 15' tall, 80,000 lbs." — so the rule's structure is used and the DOR wording is recorded rather than encoded. |
| over 15 ft high, within 16 ft wide — superload; $20 above the base fee | $20rev. · read The same $40 superload band reached on height alone, bounded at 16 ft wide for the same mutual-exclusivity reason as the $10 rows. |
| over 110 ft long, within 16 ft wide and 15 ft high — superload; $20 above the base fee | $20rev. · read The same $40 superload band reached on length alone. |
Priced by the mile. The July 2026 DOR fee sheet prints the same three per-mile bands with no weight-step table and no per-axle column.
Rounding is not a detail here: the same rate over the same miles can differ by a whole increment depending on which pounds are rounded and in which direction, so the rule is printed with the source's own words rather than applied silently.

A permit priced against an unposted-road assumption in March is simply wrong, and wrong in the expensive direction. This is the window we watch, the basis for that window in the state's own orders or statute, and the authority that posts it.

Indiana State Police publishes a rate. One officer costs at least an amount the schedule does not let us compute before mileage.
Holiday, curfew and night-travel windows are not yet encoded for Indiana. The seasonal picture — which is a different question — is in section 6 above.
They are not guessed at here. What we do publish is the seasonal weight-restriction picture, with the date we last read each state's own bulletin — a curfew invented from a plausible pattern is worth less than an honest gap, because a reader cannot tell the two apart on the page.
The agency behind each figure above is linked from that figure. We publish no per-state permit-office phone number.
We are not publishing per-state permit-office phone numbers yet, and the reason is worth stating: the two seed lists available for them are both demonstrably stale — the federal one gives two different states the same phone number — and a wrong number on a page like this costs somebody a morning. The agency behind each figure above is linked from that figure, which is the part we can stand behind today.

Named rather than gestured at, because each of these can exceed the permit itself. Every item is listed; open one for why it is excluded.
This is a permit fee, not a freight rate.
We hold no rate data, and the two commercial sites that publish market rates disagree by nearly a factor of two on the same service.
The agency's review fee is shown where the state publishes one; the engineer's own fee is a private cost with no schedule.
Set per move by the agency, and in at least one state the escort is billed at actual cost against it.
Arranged with each utility, priced by each utility.
A toll, bridge or city authority inside the same state issues its own permit, and a state total that omits it is missing a whole permit.

The single-trip base is $20, before the dimension bands, the overweight component and the transaction charge. The base alone is rarely what is paid.
The first width trigger is over 12'4".
Indiana publishes no gross-weight superload threshold in the documents on file — see the superload section, which shows the dimensional triggers it does publish.
